Job Overview

We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to individuals requiring assistance with daily activities and health management. The ideal candidate will possess a strong commitment to patient-centered care, demonstrating proficiency in medical and personal support tasks. As a Home Health Aide, you will play a vital role in enhancing the quality of life for clients by delivering personalized care, ensuring safety, and maintaining compliance with healthcare standards.

Responsibilities

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
  • Monitor and observe patient conditions, accurately documenting vital signs and changes in health status using Electronic Health Record (EHR) systems.
  • Support patients in medication administration and ensure adherence to prescribed treatment plans.
  • Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques.
  • Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively.
  • Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation.
  • Collaborate with healthcare professionals to ensure comprehensive patient care while adhering to HIPAA regulations for confidentiality.
  • Assist with hospice care needs when applicable, providing comfort and emotional support to patients and their families.

Requirements

  • Prior experience working in assisted living facilities, nursing homes, or home health environments is highly desirable.
  • Demonstrated skills in patient observation, vital signs measurement, and medical documentation.
  • Knowledge of medical terminology, medication administration protocols, and infection control practices.
  • Certification in CPR is required; additional certifications such as hospice care or dementia training are a plus.
  • Experience working with individuals with developmental disabilities or disabilities is preferred.
  • Ability to operate EMR (Electronic Medical Records) or EHR systems efficiently.
  • Strong communication skills coupled with a compassionate approach to caregiving.
