General responsibilities may include giving injections and medication, caring for wounds, recording vital signs, tracking medical history, monitoring health status, and keeping detailed documentation on patient progress.<br><br><strong>Responsibilities<br><br></strong><ul><li>Provide direct patient home care visits, observes, performs and reports preventative, treatment and rehabilitative nursing procedures while implementing the plan of care</li><li>Observe and report on the home environment and make recommendation that will enhance patient care, promote safety and prevent adverse effects</li><li>Work with patient, family and health care team members to coordinate services, promote quality of care and positive patient care outcomes</li><li>Take verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and documents appropriately</li><li>Inform Health Care Providers of changes in patient’s condition in accordance with providers orders and documents communications accordingly</li><li>Provide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort and pain interventions</li><li>Provide instructions to patient/CG’s and staff in a timely manner<br><br></li></ul><strong>Who We're Looking For<br><br></strong>You are experienced and ready to bring your skills and knowledge to patients who need it most.
