Rooted in the mission to \"go to the people\", the program offers services directly to patientson the street, in encampments, or via a fully equipped mobile medical van. The program operates as an extension of our Integrated Care Center (ICC), expanding the reach of our services beyond the clinic walls. This new and rapidly growing initiative is committed to providing low-barrier, trauma-informed, and person-centered care in the environments where people live. Our multidisciplinary team prioritizes trust, continuity, and access while delivering compassionate, dignified, and high-quality services to some of the city\'s most underserved populations.</p><p>The Nurse Manager will act as a clinical leader, providing key nursing support to patients in this unique setting. This role involves promoting team functioning and efficiency, managing and supporting staff, championing quality improvement, and leading change. The Mobile Medicine & Street Medicine nurse manager will provide direct patient care on the street to patients experiencing homelessness. Additionally, they will work closely with the Program Director to develop and implement policies and procedures, establish best practices, and create efficient systems to ensure high-quality patient care.</p><h2>Key Responsibilities</h2><p>Clinical Care Responsibilities:</p><ul><li>Provide nursing care as part of an integrated care team, within the scope of license and agency credentials, to patients directly on the street or in encampments, from a mobile medical van, and at HR360\'s brick and mortar site, the Integrated Care Center (ICC) at 1563 Mission Street in San Francisco.</li><li>Provide complex care management for patients with multiple chronic medical illnesses and behavioral health concerns.</li><li>Is the Enhanced Care Management (ECM) lead on the Mobile Medicine and Street Medicine team.</li><li>Documents care promptly according to charting protocols, applicable California state licensing requirements, and clinic policies in the electronic medical record (EMR).</li><li>Utilize motivational interviewing skills and encourage patient activation and self-management.</li><li>Advocates for patients experiencing homelessness, ensuring they receive fair and comprehensive healthcare services, and represent their needs within the broader healthcare system and with social service agencies.</li><li>Appropriately utilize standing orders.</li><li>Facilitate team functioning and efficiency by taking responsibility for additional patient assessments or medical assistant functions (e.g., phlebotomy, rooming, vitals, etc.) to ensure the completion of patient sessions in a timely and thorough manner.</li><li>Assess and provide nursing advice to patients when indicated.</li><li>Provide thorough patient education when indicated.</li><li>Respond to patient requests for information and assistance (e.g., form completion, prescription refills, etc.) in a timely manner.</li><li>Inform clients of normal and abnormal lab results and follow-up plans, as appropriate for scope or under guidance of a provider.</li><li>As a part of the care support team, prioritize and take responsibility for patients with complex medical issues and follow up with all patients regarding recent emergency room visits and inpatient stays.</li><li>Work collaboratively with mental health staff and other ICC services to facilitate integration between primary care, mental health, dental services, and other ancillary services to improve patient outcomes.</li><li>Coordinate and oversee the timely reporting and coordination of care of public health reportable diseases.</li><li>Manages alignment of the scheduling and logistics for lab specimen collection and delivery with the ICC medical clinic.</li><li>Oversee the delivery and secure storage of medications.</li><li>With assistance from the administrative coordinator and medical assistant, manages the procurement of supplies, maintains and evaluates inventory lists, and ensures supplies are adequately stocked.</li><li>Obtains and maintains licensure and BCLS/CPR certification.</li><li>Attend trainings and conferences related to the Street Medicine field, as directed by Mobile Medicine and Street Medicine director.</li><li>Attend continuing professional education programs as required for certification/licensure.</li></ul><p>Leadership Responsibilities:</p><ul><li>Provides clinical leadership to support staff, including other nurses, MAs, case managers, and community health workers to deliver high-quality patient care. Provide ongoing recognition, feedback, and support to develop the team\'s professional skills.</li><li>Acts as the day-to-day clinical operations lead on the Mobile Medicine & Street Medicine team, with assistance from the Manager and Director of Operations at ICC and in close collaboration with the Mobile Medicine & Street Medicine Director.</li><li>Assist the Director in strategic planning and program development.</li><li>Work collaboratively with providers and other staff to develop standardized nursing protocols and standing orders.</li><li>Participate in training other nurses, medical assistants, case managers, community health workers, learners, students, and volunteers.</li><li>Participate as an active member of the Care Support Team through case-conferencing and other collaborative work.</li><li>Demonstrate comfort in changing systems, champions change and supports change processes.</li><li>Demonstrate commitment to the mission of the organization, the healthcare of medically underserved people, harm reduction, client centered care, racial justice, and community-oriented primary care.</li></ul><p>Performance and Quality Improvement Responsibilities:</p><ul><li>Serve as an active participant in Quality Improvement activities.</li><li>Work closely with staff collecting and reporting contractual data as necessary.</li><li>Meet benchmarks for applicable quality indicators as noted in the annual quality management plan including the maintenance of current productivity standards, cycle times, no-show rates, and landing times.</li><li>Along with colleagues, assists in continuous quality improvement through on-going problems/needs identification and problem solving and as requested, acts as change champion or lead in improvement activities.</li><li>Maintains quality assurance logs and leads staff to ensure the quality of medications, supplies, medical equipment, and laboratory testing.</li></ul><p>Organizational Responsibilities:</p><ul><li>Keep up to date with operational and procedural requirements.</li><li>Adhere to company dress code and maintains professional appearance.</li><li>Complete mandatory training within deadlines.</li><li>Maintain availability for various administrative/clinical duties as requested by the supervisor (e.g., case presentations).</li><li>Provide in-service training for staff as needed.</li></ul><h2>Qualifications</h2><p>Education, Credentials, and Experience:</p><ul><li>Graduation from an accredited school of nursing.</li><li>Registered Nurse License by the California Department of Consumer Affairs (DCA).</li><li>Possession of a valid California license as a registered nurse.</li><li>First Aid & CPR certification.</li><li>Valid driver\'s license and ability to travel to various locations within San Francisco.</li><li>A minimum of 3 years of direct patient care experience in ambulatory care, community health center, emergency medicine, public health, in Street Medicine, and/or in mobile medicine.</li><li>A minimum of 1 year of leadership/supervisory experience, ideally in a community healthcare setting.</li><li>Experience working successfully with issues of homelessness, substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.</li><li>Experience working with populations with varying lifestyles, ages, sexual orientations, ethnic and culture backgrounds and economic status.</li><li>Some experience in case management, ideally enhanced care management (ECM).</li></ul><p>Desired:</p><ul><li>5 years of direct patient care experience in ambulatory care, community health center, emergency medicine, public health, in Street Medicine, and/or in mobile medicine</li><li>3 years of leadership experience in a community healthcare setting; preferred experience in leading other RNs in a multidisciplinary setting.</li><li>Significant experience working in ECM.</li><li>Experience</li></ul></div>
