Any information collected there is subject to their terms and privacy notice.<br><br>POSITION TITLE: OCI Peer Assistant Case Manager<br><br>FLSA CLASSIFICATION: Non-Exempt<br><br>REPORTS TO: Sr. Director of Adults and Children's Services, Coordinator of Operation Community Integration<br><br>POSITIONS SUPERVISED: N/A<br><br>POSITION OVERVIEW: The Peer Assistant Case Manager is responsible for providing Community Psychiatric Support & Treatment and Peer Support services to adult consumers enrolled in Intensive Case Management, Community Integration. The Peer Assistant Case Manager will work with consumers and ancillary service providers to assist the persons served in meeting identified goals, with the overall objective of maintaining the consumer in the community at the least restrictive level. The Peer Assistant Case Manager is responsible for utilizing his/her own recovery story to help consumers to develop the skills necessary for recovery.<br><br><strong>Essential Position Responsibilities<br><br></strong><ul><li> Provides services to maintain required productivity/billing standard of 5 contacts a day. </li><li> Meets deadlines and ensures accuracy of various reports/paperwork, electronic mileage entries, and electronic timesheets. </li><li> Provides peer support (PSI) services in accordance with the consumer’s treatment plan goals. </li><li> Maintains accurate and timely documentation of service provision per MHA, MCO/Medicaid, and COMCARE guidelines and contracts. Completes treatment plans, strengths assessments, EBP documents, releases, and progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals. </li><li> Assists consumers in communicating, setting, and achieving goals and objectives for recovery identified on their individual treatment plan. </li><li> Assists consumers in obtaining or sharing information in a group or individual (one-on-one) setting to aid in the recovery process. Facilitates Peer Support Groups on a fill-in basis to meet departmental needs. </li><li> Assists and teaches consumers to communicate, advocate, and make informed choices in all areas of their lives which include, but are not limited to medications, diagnosis, treatment, housing, employment, and education. </li><li> Models effective coping skills and self-help strategies. </li><li> Makes referrals to ancillary services as appropriate per consumer need. </li><li>Collaborates with care providers to coordinate services required to meet identified goals based on the needs of the consumers and congruent with the consumer’s level of functioning. </li><li>Ensure consistent medical coverage for consumer & assist in completion of reviews as appropriate. </li><li>Assists other case managers on OCI team as needed to ensure continuity of consumer care. This includes providing coverage to cases while other team members are on leave or on a fill-in basis if a team member is unavailable. </li><li>Engages in active outreach attempts per departmental need to re-engage consumers in services to work toward recovery goals. Outreach may consist of but is not limited to the following activities: attempting to make face to face contact with consumers at their residences, accessing homeless shelters to locate consumers, and/or contacting consumers’ emergency contacts to establish engagement. </li><li>Works in collaboration with OCI Coordinator for comprehensive care and quality outcomes. <br><br></li></ul><strong>Other Position Requirements<br><br></strong><ul><li> Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and training as required. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible in work schedule when needed. </li><li> Exhibits appropriate level of technical knowledge for the position. </li><li> Produces quantity of work necessary to meet job requirements. </li><li> Works well with a team, keeps others informed of information needed. Treats others with respect, maintaining a spirit of cooperation. </li><li> Maintains effective and professional verbal and written interactions with peers, customers, supervisors and other staff. Uses diplomacy and tact in dealing with difficult situations or people. Demonstrates effective listening skills. Is receptive to constructive feedback. </li><li> Demonstrates the ability and willingness to handle new assignments, changes in procedures, and business requirements. Identifies what needs to be done and takes appropriate action. </li><li> Completes assigned work, meets deadlines without reminders/follow-up from supervisor or others. </li><li> Performs work conscientiously with a high degree of accuracy. <br><br></li></ul>POSITION REQUIREMENTS: Applicants must possess a Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. The applicant must also be a Certified Peer Specialist or be able to meet requirements to become a Certified Peer Specialist within the first six months of employment. The position requires the applicant to self-identify as a present or former primary consumer of mental health services. Computer experience required. Preferred experience includes knowledge of community resources, experience working in the community with mentally ill adults; ability to write and communicate verbally in a clear and concise fashion; and the ability to develop and maintain rapport with consumers, families, & other community providers. A valid Kansas drivers license and access to personal vehicle required.<br><br><strong>Physical Requirements<br><br></strong><ul><li> Driving (for purposes of community mobility) </li><li> Data entry, writing/note taking </li><li> Lifting/carrying up to 30 pounds </li><li> Bending/Stooping/climbing stairs </li><li> Typing for extended periods of time </li><li> Sitting for extended periods of time <br><br></li></ul>All the above duties and responsibilities are considered essential job functions subject to reasonable accommodation.
