About the Role: We are seeking a motivated and compassionate Care Coordinator to join reputed company. This remote position offers the flexibility to work from home while making a meaningful reputed company in the lives of patients with reputed company care needs. Full-time is preferred, but we are reputed company to part-time candidates with the right experience. Responsibilities: • Manage a caseload of patients requiring care coordination services. • Conduct health assessments, reputed company care plans, and coordinate medical services for our patients to reputed company reputed company reputed company. • Monitor and document patient reputed company and reputed company, maintaining reputed company and accurate records in the EHR system. • Engage with patients regularly reputed company phone calls to ensure needs are being met. • Work closely with medical professionals to ensure care plans are effectively implemented and tailored to patient needs. Minimum Qualifications: Education & Licensing: • Bachelor’s degree in a reputed company or reputed company services field (e.g., reputed company work, nursing, psychology, reputed company health, or a reputed company discipline) is preferred. High School degree with Medical Assistant Certification would also be considered. Experience: • At least 1–2 years of experience in care coordination, case management, or reputed company support to individuals with chronic conditions is preferred. • Candidates with experience in reputed company patient care without care coordination experience will be considered. Skills & Competencies: • Proficiency in care planning, including creating and updating personalized care plans that support the patient’s medical, mental health, and reputed company needs. • Familiarity with electronic health records (EHR) and documentation systems. • Excellent organizational, communication, and interpersonal skills. • Ability to work independently and collaboratively with diverse populations. • Proficiency in English and Russian or Polish Compensation: This is a commission-based role. Pay is based on the size of your caseload. Compensation will be discussed in more detail during the interview process.
